Change of name on the register

To change your name on the register, you must provide the Board with:

  1. A written request signed by the nurse or midwife to include:
    • request for change of name on register identifying current full name and preferred name on register.
    • the Division(s), Speciality(s) and Registration number of current registration with the Nurses and Midwives Board of Western Australia.
    • date of birth
  2. A certified copy of your change of name document, i.e. Marriage Certificate, Deed Poll or Change of Name Licence. If you are attaching the original document, please provide a prepaid registered post return mail envelope for return of your original change of name document.

Change of address on the register

The following options are available to change your address on the register:

  1. A written request from the nurse or midwife to include:
    • request for change of address on register identifying current or most recent address on the Register and new address;
    • the Division/s, Specialities and Registration number of current registration with the Nurses and Midwives Board of Western Australia; and
    • date of birth; or
  2. Completion of the Change of Address section on a computer generated Renewal of Registration form; or
  3. There is the provision to change your address on the sheet provided with your OnBoard Journal; or
  4. Request from the nurse or midwife to change address via email:
    • Request to change address details on the register, identifying current or most recent address on the register and the new address;
    • Full name of nurse or midwife;
    • Date of birth of nurse or midwife;
    • The Division/s, Specialities and Registration number of current registration.
