How to Make a Complaint

If you are concerned that a nurse or midwife is not providing safe or competent care, or that their behaviour is not as professional as you would expect you may make a complaint in writing to the Director, Professional Governance stating the nature of the complaint and identifying the nurse or midwife involved.

Please refer to the form and guidelines at the end of this page

What Information Do I Need to Provide?

Along with an Authority to Release and a Statutory Declaration, you need to provide as much information as you can including the full name of the nurse or midwife, details of the incident, times, places and any other witnesses.

Where possible, please forward any documented evidence such as statements, anecdotal notes and relevant reports that substantiate your complaint to NMBWA.

In investigating your complaint it is necessary to release a copy or details of your complaint to the nurse or midwife who is the subject of that complaint or to others to facilitate the investigation. However, no further details other than your name will be released. In addition, your complaint may be used as evidence in an inquiry. Please contact the Director Professional Governance on (08) 94 211 100 if you have particular concerns in relation to the release of your details.
