To change your name on the register, you must provide NMBWA with:
1. A written request signed by the nurse or midwife to include:
- request for change of name on register identifying current full name and preferred name on register
- the registration number of current registration with NMBWA
- your date of birth
- category of registration (eg. enrolled nurse, registered nurse, nurse practitioner, midwife) .
2. A certified copy of your change of name document ie. marriage certificate, deed poll or change of name licence. If you are attaching the original document, please provide a prepaid registered post return mail envelope for return of your original change of name document.
3. A fee of $20 is applicable.
The following options are available to change your address on the register:
1. A written request from the nurse or midwife to include:
- request for change of address on register identifying current or most recent address on the Register and new address
- Category and registration number of current registration with NMBWA
- date of birth.
2. Completion of the Change of Address section on a computer generated Renewal of Registration form.
3. Request from the nurse or midwife to change address via our change of address web form.
