On successful completion of an accredited nursing and / or midwifery course leading to registration, the student is eligible to apply to the Board for registration as a nurse and / or midwife.
The following documentation is required in order for an application can be considered for registration:

Statement of Student Eligibility for Registration (must be sent directly to the Board by the Education Provider).
To be provided by the student:
Application form
Application fee
Proof of Identity documents (birth certificate or passport)
Change of name document (if applicable)
Final Academic Statement
Evidence of English Language Proficiency (if applicable - see policy below)
